I confirm that I am at least 18 years of age and legally able to receive a tattoo in the state of Pennsylvania.

Voluntary Consent
I acknowledge a tattoo is permanent body modification and transformation and I am receiving this tattoo by my own choice and will not hold the artist or studio liable for any decision I make during or after the procedure.

Health Disclosure
Ihave disclosed any medical conditions, skin sensitivities, blood-borne illnesses, or other health issues that may affect the tattoo process or healing.

Aftercare Responsibility
I understand that proper aftercare is crucial and accept full responsibility for the healing of my tattoo after I leave the studio.

Photo & Video Release

TATTOO AFTERCARE GUIDE

Proper aftercare is essential to ensure your tattoo heals cleanly and maintains strong contrast and detail over time. Follow these instructions carefully during the healing process.

FIRST 24 HOURS

• Leave your bandage or wrap on for the amount of time recommended by your artist. • Wash your hands before touching your tattoo. • Gently wash the tattoo with warm water and mild, fragrance-free soap. • Pat dry with a clean paper towel — do not rub. • Apply a thin layer of recommended aftercare ointment or lotion. • Do not rewrap unless instructed.

DAYS 2–14

• Wash tattoo 2–3 times daily. • Apply a thin layer of fragrance-free lotion after washing. • Wear clean, loose clothing over the tattoo. • Expect light peeling or flaking — this is normal. • Do not scratch or pick at healing skin.
AVOID DURING HEALING

• Swimming, baths, or soaking the tattoo • Direct sunlight or tanning • Heavy friction or tight clothing • Scratching or shaving the area • Excessive sweating when possible
NORMAL HEALING SIGNS

Healing tattoos may: • Peel or flake • Feel itchy • Look cloudy or dull temporarily • Shed small bits of inked skin
Color and clarity return once healing finishes.
Showers are fine, but avoid soaking.
Reach out if you notice: • Excessive redness spreading outward • Severe swelling • Yellow/green discharge • Fever or unusual pain
